Level I Triage Category

Respiratory (RESP)
  • Airway compromise
  • Severe respiratory distress, inadequate breathing
  • Critical asthma
  • Chest trauma with respiratory distress
     
Neurological (CNS)
  • Major head injury
  • Unconscious or unresponsive
  • Active seizure state
     
Cardiovascular (CVS)
  • Cardiac arrest
  • Shock or hypotension
  • Exsanguinating hemorrhage
     
Musculoskeletal (MSK)
  • Major trauma
  • Traumatic amputation -- extremity
  • Major cold injury -- hypothermia
     
Skin
  • Burn, >25% body surface area (BSA) or airway involvement
     
Gastrointestinal (GI)
  • Difficulty swallowing, with airway or respiratory compromise
  • Abdominal trauma -- penetrating or blunt -- signs or symptoms of shock
     
Gynecological (GYN)
  • Vaginal bleeding, patient with abnormal vital signs
     
Hematologic/Immunologic
(HEM/IMMUN)
  • Anaphylaxis

Level II Triage Category

RESP
  • Marked stridor
  • Moderate respiratory distress
  • Severe asthma
  • Foreign body aspiration with respiratory distress
  • Inhalation of toxic substances
     
CNS
  • Moderate head injury with altered mental state
  • Altered consciousness
  • Shunt dysfunction -- patient appears ill
  • Sudden onset of confusion, weakness, severe headache
     
CVS
  • Significant tachycardia
  • Bradycardia
  • Severe dehydration
  • Uncontrolled major hemorrhage
     
MSK
  • Traumatic amputation -- digit
  • Fracture, open or with neurovascular impairment
  • Back pain with neurologic deficit
  • Avulsed permanent tooth
     
Skin
  • Burn, >10% BSA or face, hand or foot involvement
  • Burn, chemical or electrical
  • Purpuric rash
     
GI
  • Acute bleeding, vomitus or rectal
  • Abdominal pain with vomiting, diarrhea, or abnormal vital signs
     
Genitourinary (GU)
  • Severe testicular pain
  • Urine retention >24 hours
  • Paraphimosis, priapism
     
GYN
  • Possible ectopic pregnancy with normal vital signs
  • Severe vaginal bleeding
     
Eye/ear/nose/throat (EENT)
  • Chemical substance in eye
  • Burn, penetration, of eye
  • Orbital infection
  • Impaled object or amputation of ear
  • Uncontrolled epistaxis / post T&A bleeding
  • Sore throat with drooling, stridor, or difficulty swallowing
  • Hoarseness, sudden onset with history of trauma to larynx
     
HEM/IMMUN
  • Bleeding disorder
  • Fever -- neutropenia / sickle cell disease
     
ENDO
  • Diabetic -- ketoacidosis / hypoglycemia
     
Psychiatry (PSYCH)
  • Toxic overdose
  • High risk of harm to self or others
  • Violent behaviour

  • Infant <7 days old

  • Any infant or child with toxic appearance
  • Infant <3 months, temperature <36° or >=38°C

Level III Triage Category

RESP
  • Stridor
  • Mild respiratory distress
  • Moderate asthma
  • Foreign body aspiration, cough present, with no distress
  • Constant cough, appears distressed
     
CNS
  • Minor head injury, GCS ¾15
  • History of altered consciousness
  • Headache
  • Possible shunt obstruction with no distress
  • Seizure prior to emergency visit, not actively seizuring
     
CVS
  • Tachycardia
  • Signs of dehydration
  • Uncontrolled minor hemorrhage
     
MSK
  • Probable fracture with no neurovascular deficit
  • Tight cast with possible neurovascular impairment
  • Joint pain with fever
  • Dental trauma
     
Skin
  • Burn, partial thickness and <10% BSA
  • Burn, full thickness <5% BSA
  • Localized cold injury
  • Cellulitis -- patient appears ill, or is febrile
  • Complex lacerations
     
GI
  • Persistent or bilious vomiting
  • Vomiting or diarrhea <2 years
  • Possible appendicitis
     
GU
  • Moderate testicular pain or swelling
  • Inguinal bulge with pain
  • Urinary retention >8 hours
     
GYN
  • Vaginal bleeding with normal vital signs
     
EENT
  • Foreign body in nose causing pain or possible risk of aspiration
  • Epistaxis, controlled / history of post T&A bleeding
  • Puncture wound of soft palate
  • Tonsil pustules with difficulty swallowing
  • Hearing problem -- acute onset
  • History of postoperative bleeding -- tonsillectomy or adenoidectomy
  • Foreign body in ear
  • Periorbital swelling with fever
  • Sudden vision change
     
HEM/IMMUN
  • Sickle cell crisis
  • Moderate allergic reaction
     
ENDO
  • Diabetic -- Hyperglycemia
     
PSYCH
  • Ingestion requiring observation
  • Moderate risk of harm to self or others
  • Disruptive or distressed
     
Behaviour
  • Unconsolable infant
  • Infant not feeding
     
INF
  • Infant 3­36 months, temperature >38.5°C
     
Child abuse
  • Physical assault
  • Sexual assault <48 hours

Level IV Triage Category

RESP
  • Mild asthma
  • Possible foreign body aspiration with no history of distress
  • Minor chest injury with no respiratory distress
     
CNS
  • Minor head injury -- no vomiting or altered consciousness, GCS = 15
  • Chronic or repeating headache with no acute distress
     
CVS
  • Chest pain with normal vital signs
     
MSK
  • Possible extremity fracture, green-stick or buckle
  • Sprain or strain
  • Extremity swelling
     
Skin
  • Minor cold injury -- no discolouration -- minimal pain
  • Localized cellulitis
  • Minor burn
     
GI
  • Constipation; not eating; cramps
  • Abdominal pain with vomiting or diarrhea >2 years old
     
GU
  • Scrotal trauma
  • Possible urinary infection
     
EENT
  • Corneal foreign body or abrasion
  • Crusting, matting, discharge from eye
  • Ear drainage
  • Earache
     
HEM/IMMUN
  • Local allergic reaction

  • Sore throat, laryngitis, minor mouth sores
  • Nasal congestion, allergy or upper respiratory infection
  • Conjunctivitis
     
PSYCH
  • Chronic symptoms with no acute changes
